Position Summary: The Intake Supervisor is responsible for the management of the intake team along with all aspects of the patient intake process including but not limited to collecting appropriate patient documentation from referral sources, ensuring a smooth and timely transition to home health services, and establishing and maintaining effective relationships with all stakeholders involved. This role works under the direct supervision of the Senior Director of Operations, collaboratively ensuring the overall coordination of incoming referrals is smooth, efficient and customer focused.

    Responsibilities:

    • Supervises the daily operations and performance of the Intake Coordinators including workflow management, training, hiring and disciplinary actions
    • Responsible for the coordination and scheduling of the intake staff to ensure proper levels of coverage including weekends and holidays
    • Directs all daily patient referral and intake operations including providing direct oversight of the intake policies and procedures
    • In collaboration with the care coordination team, ensures a seamless and timely transition of patients into HealthFlex services
    • Consults with the Regional Executive Director on issues that require escalation for clinical judgment as needed
    • Facilitates routine, team and one on one meetings with staff to provide ongoing communication and training
    • Attends and participate in daily huddle meetings
    • Creates and maintains reports tracking KPIs
    • Ensures compliance with all state, federal, and accrediting body referral/intake regulatory requirements
    • Addresses customer complaints and escalates to the Senior Director of Operations as needed
    • Proactively provides problem resolutions and performs ongoing quality checks and process improvement initiatives in order to streamline workflow
    • Establishes and maintains positive relationships with current and potential referral sources and other stakeholders such as patients and their families, care coordinators, clinical supervisors and account representatives.
    • Maintains a clear understanding of staffing capacity for all disciplines in order to service the appropriate levels of new patients
    • Maintain extensive knowledge of the regulatory guidelines governing home health admissions
    • Presents HealthFlex as the provider of choice and the benefits of home health services
    • Performs direct Intake Coordinator duties as needed
    • This position has oversight of multiple office locations and will require some travel

    Requirements:

    • Current and unrestricted California Licensed Vocational Nurse license
    • Minimum 3 year experience of Home Health Intake/Admissions experience
    • Minimum of 1 year of experience in a leadership capacity
    • Strong working knowledge of regulatory guidelines governing home health admission

    Office hours Monday-Friday 9am-5pm
